Preparing For Your Visit

Your first appointment will be a new patient comprehensive exam, during which we will discuss with you your dental concerns and needs. Please reserve 1 hour for this appointment. Should you need to reschedule, kindly notify us 2 business days in advance.

To allow us to better serve you, please print and complete these forms and bring with to your first appointment.

Your First Appointment

  1. Please arrive 15 minutes before your scheduled appointment to allow sufficient time for the check-in process.

  2. Please complete and bring the new patient forms available for download.

  3. Bring a list of all medications, prescription and over-the-counter, you are currently taking.

 

Your initial visit will consist of a clinical exam and a review of your medical and dental history.  A clinical exam will allow us to evaluate your current dental conditions, screen for oral cancer, examine your teeth, gum tissue, jaw joints and facial muscles. We look at the entire person, not just an individual tooth.

 

We will then address any questions or concerns you may have regarding your dental condition. Together we will look at all the data gathered from your exam and tailor a treatment plan that best fits your individual needs.

If you have been referred by your current dentist, we will be in communication with them to coordinate efforts in your treatment.

Payment & Insurance 

Due to the nature of our specialty practice, it is our policy to have payment in full at the time of service. You can pay for your office visits and any treatments by cash, check, credit or debit card at the time of treatment.

 

We believe that you have the right to choose your dentist, regardless of your insurance coverage.  Our office accepts all PPO insurance plans, but is not contracted directly with any insurance company.  We are considered an out-of-network provider. Because we are a specialty practice, the majority of our procedures fall into the category of elective dentistry; however, we are happy to submit claims for you and ensure your insurance provider pays you the maximum amount they have contractually agreed upon for covered procedures.  The financial obligation for dental treatment is between you and our office.  The insurance company is responsible to you directly. To discuss your payment options or insurance coverage, please call us at 425-453-1117.
